Complete Application form
Copy of High School Diploma/ GED certificate
Submit to a Fingerprint-based Health Care Worker Criminal History Records Check
TB skin test or Chest X-ray which is less than a year old
Practical Nursing Program Requirements:
Complete the Practical Nursing Program Application form
Copy of High School Diploma/ GED certificate.
Completion of a Nursing Assistant Program
Pass the TEAS Exam
Submit to a Fingerprint-based Health Care Worker Criminal History Records Check
Current CPR certification
Complete Physical Examination
Medical Insurance
